REGISTERED IN THE HOLDER'S NAME BE SUBJECT TO REPAYMENT ON JULY 23, 2004, THE
HOLDER OR THE HOLDER'S AUTHORIZED ATTORNEY MUST DELIVER TO THE TRUSTEE, AND THE
